| Exams must be on file prior to starting summer band in August. Exams must given by a physician or licensed physician's assistant. The necessary forms to download may found on the CFISD website www.cfisd.net Go to departments then athletic department. Physical Examination Form. please note that these forms will be passed out in classes the week of April 28th. The only form that is needed is the form that the doctor fills. The forms on the cfisd website for UIL and pernission are not needed for band for this, we have our own forms for band. |
